Medicare Advantage Plans

 

Your Chamber offers two group Medicare Advantage (Part C) plans:

• CDPHP PPO $10/$15 Plan

• MVP Preferred Gold HMO-POS Plan

 

Both offerings include Part D Prescription coverage.

Enrollment Requirements:

• To qualify for enrollment in either plan, you must already have Medicare Part A and B.

• CDPHP requires a subscriber to be retired. MVP Healthcare subscribers do not need to be retired.

 

NOTE:  Sole Proprietors or Individuals are not eligible for Medicare group products.
